Employer's Initial Report of Injury (E1) for Work-based Learning Placements
(Advanced Education) This form is used by an employer to advise the WCB of an injury to a worker from a Work-based Learning Placement. The Employer's Initial Report of Injury (E1) must be submitted to the Ministry of Advanced Education, Employment and Labour within three days. This form should be completed by the institution's/community-based organization's work based learning co-ordinator in consultation with the employer. |
