APPENDIX A Forms for Elections in Cities, Towns, Villages, Resort Villages, Northern Municipalities and School Divisions and on Indian Reserves have been named as L-30.1 Reg 4 Form A, L-30.1 Reg 4 Form B, etc.
APPENDIX B Forms for Elections in Rural Municipalities have been named as L-30.1 Reg 4 Form A-RM, L-30.1 Reg 4 Form B-RM, etc. to distinguish the difference between them.
APPENDIX C Forms for Voter Identification have been named as L-30.1 Reg 4 Form A-VI, L-30.1 Reg 4 Form B-VI, and L-30.1 Reg 4 Form C to distinguish the difference between them.
